I am aware of the nature of the activities and declare that I am physically fit and able to participate. I release Dojos Ken USA and its staff from any liability in case of injury or accident during the class.

I hereby grant Dojos Ken USA, Inc., and its affiliates permission to use, reproduce, display, and distribute any photographs, videos, or likenesses of me, my child/children for promotional purposes, including socal media, webste, blogs, articles, and other marketing materials. I understand that I wll no receive compensation for this. I also acknowledge that Dojos Ken USA, Inc., will make efforts to maintain the privacy of my personl information.
